The Downtown Women’s Center seeks a full time Rapid Re-Housing Case Manager (RRH CM) to work with the CES Rapid Re-Housing Program (CES-RRH) targeting single adults who are homeless to secure permanent housing and appropriate supportive services to retain housing. The RRH Case Manager serves as the main entry point for the CES- RRH program and will be integral in identifying eligible clients. Once intake and eligibility is complete, this position ensures clients have the needed financial resources to secure permanent housing. This position is part of an interdisciplinary team and provides innovative services that are at the forefront of homeless services. An ideal candidate is strengths-based, decisive, flexible, and creative. This position is a member of the Community Based Housing department and reports directly to the Rapid Re- Housing Program Manager. Essential Functions Complete CES packet (triage tool) on all clients identified to determine most appropriate level of housing intervention (rapid re-housing or permanent supportive housing) Identify and assist clients in securing housing, basic needs, medical and mental health services, substance abuse treatment when necessary, job training access, and public benefits where eligible Provide strengths-based case management and service coordination designed to assist clients in obtaining and maintaining stable housing Utilize evidence based practices in service delivery such as Trauma-Informed Care, intensive case management, Motivational Interviewing, Harm Reduction, Critical Time Intervention, and Housing First practices Maintain accurate and timely records of activities, case management notes, and services provided to each client using HMIS Respond to general information inquiries received via phone, email, and in person Develop and maintain relationships with clients, staff and local service providers, participate in community meetings pertaining to services benefiting clients, and serve as a representative of DWC at community events Provide regular, written reports to the Rapid Re- Housing Program Manager and additional reports as assigned Other duties as assigned to support the mission of the organization QUALIFICATIONS Necessary Skills/Qualifications Bachelor’s degree in social work or related field At least two years of experience as a case manager or similar position and knowledge of community resources required Strong knowledge of the complexity of poverty and women’s homelessness, mental illness, trauma, substance abuse, health issues, and older adults preferred Ability to manage multiple tasks and priorities, including proficiency in balancing direct service responsibilities with effective record keeping Ability to problem-solve and navigate complex systems; must possess effective negotiation and conflict resolution skills Excellent active listening and crisis intervention skills with ability to exercise and model good judgment Strong interpersonal, written and oral communication skills; proven ability to communicate with diverse audiences Strong administrative skills and proficiency in Microsoft Office Suite (Word, Excel, Outlook) required, and HMIS preferred Criminal background check and clearance required Valid California state driver’s license, insurance, and use of a reliable vehicle required Schedule & Salary : This is a full-time, non-exempt position, scheduled Monday – Friday, 9:00 AM – 5:30 PM; with flexibility to work during non-scheduled days and evenings when necessary. Salary $38,000 annually depending on qualifications & experience.
